I have to enforce and observe physical distancing practices, and personal protective equipment is available on request and as required when we’re not able to maintain a 2 meters distance. I will be asking you to wash your hands with soap, or use alcohol-based sanitizer upon entry, and wear a mask (unless you have a condition that inhibits your ability to wear a mask or face covering). As always, I will have regular hand washing or sanitizing protocols before and after appointments.
THE MOST IMPORTANT THINGS THAT YOU SHOULD NOTE:
- Please fill out the Covid-19 waiver (below) prior to your next in-office appointment. This form will include more details on clinic practices during this public health crisis.
- I will be contacting you via email and/or phone to screen you for any potential respiratory viral infections 24hrs in advance of your appointment, and to set up touchless payment. If you are experiencing any symptoms, or have traveled internationally in the last 14 days, as per provincial mandates, we’ll be asking you to reschedule your appointment or meet via telemedicine.
- We’re asking that you wait in your car before entering the clinic until your appointment time. Upon entering I will confirm that you’ve answered the screening questions.
